چکیده

Background Myocardial strain analysis has been proposed as a surrogate for regional replacement fibrosis (scar) in patients with ischemic cardiomyopathy (ICM). However, contractile function is often degraded in non-scarred tissue, conceivably due to a composite of interstitial fibrosis, metabolic aberrations and abnormal electro-mechanical coupling. We tested a novel 4D strain analysis tool to examine strain characteristics of scarred and nonscarred myocardium in patients with advanced ICM.

4D strain analysis within non-infarcted myocardium of patients with ischemic cardiomyopathy: potential marker for the prediction of adverse cardiac events

Background Adverse left ventricular remodeling following myocardial infarction (MI) remains one of the strongest prognostic indicators of adverse cardiac events. Increased wall stress and biomechanical strain within non-infarcted myocardium contribute to myocyte hypertrophy, fibrosis, and late adverse remodeling. This study aimed to demonstrate the feasibility of performing 4D strain analysis w...

Free-breathing 3D phase-sensitive inversion recovery late gadolinium enhancement at 3.0 Tesla: reliability and image quality in ischemic and non-ischemic cardiomyopathy in comparison with multiple breath-hold 3D imaging

Background In both ischemic (ICM) and non-ischemic (NICM) cardiomyopathy late gadolinium enhancement (LGE) is an important cardiovascular magnetic resonance (CMR) technique. LGE CMR, traditionally performed in 2D during multiple breath-holds (MB), is challenging for vulnerable patients and subject to slice misregistration. Therefore, LGE CMR during free-breathing (FB) is more robust and enables...

Microvascular ischemia in hypertrophic cardiomyopathy: new insights from high-resolution combined quantification of perfusion and late gadolinium enhancement

BACKGROUND Microvascular ischemia is one of the hallmarks of hypertrophic cardiomyopathy (HCM) and has been associated with poor outcome. However, myocardial fibrosis, seen on cardiovascular magnetic resonance (CMR) as late gadolinium enhancement (LGE), can be responsible for rest perfusion defects in up to 30% of patients with HCM, potentially leading to an overestimation of the ischemic burde...

Refining the characterization of residual function in hypertrophic cardiomyopathy through remote segment 4D strain analysis

Background Quantitative assessment of late gadolinium enhancement (LGE) by cardiovascular magnetic resonance imaging (CMR) has been associated with an increased risk of sudden cardiac death (SCD) in patients with hypertrophic cardiomyopathy (HCM). However, patients with lesser degrees of LGE may still remain at high risk of adverse cardiac events due to the diffuse pathophysiology of HCM. Non-i...
